Description:
ForWARD Health is seeking a compassionate and skilled Certified Nurse Assistant to join our team. The nurse assistant is a paraprofessional member of the home care team who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient’s personal needs, to promote the patient’s comfort, and a safe environment. The nurse assistant is responsible for observing patients and reporting/documenting these observations and the services provided.
Responsibilities:
Responsibilities of the nurse assistant include, but are not limited to, the following:
Provident personal care including:
Baths
Back rubs
Oral hygiene
Shampoos
Changing bed linen
Assisting the patient with dressing and undressing
Skin care to prevent breakdown
Assisting the patient with toileting activities
Keeping a patient’s living area clean and orderly.
Planning and preparing nutritious meals.
Assisting in feeding the patient, if necessary.
Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
Assisting in ambulation and exercise according to the plan of care.
Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
Assisting patient in the self-administration of medication.
Doing patient’s laundry, as appropriate.
Meeting safety needs of patients and using equipment safely and properly (foot stools,
side rails, etc.)
Reporting on patient’s condition and significant changes to the assigned supervisory nurse.
Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct. Requirements:
Valid certification as a Certified Nursing Assistant in Colorado.
Basic Life Support (BLS) through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire. All life support certification cards must be issued by either the American Heart Association or the American Red Cross CPR for the Professional Rescuer. (American Red Cross for the Professional Rescuer is only good upon hire and all renewals will need to be through an American Heart Association sanctioned course.)
Ability to read and follow written instructions and document care given.
Displays initiative and is able to work with minimal direct supervision.
Empathy for the needs of the ill, frail, and impaired.
Demonstrates tact, patience, and good personal hygiene.
Licensed driver with automobile that is insured in accordance with the Organization requirements and is in good working order.
Possess and maintain valid CPR certification, if required.
Qualifications:
Strong interpersonal and communication skills, with the ability to collaborate effectively within a team.
Compassionate, patient-centered approach to care delivery.
Ability to work independently and make sound clinical judgments.
Subject to background check and drug screening.
Stay up to date with Axxess (electronic medical record) training and qualifications.
Must be able to lift, pull, push and carry 25 pounds; must be able to bend, twist, stoop, kneel and reach.
Job Types: Full-time, Part-time, PRN
Pay: $20.00 - $25.00 per hour
Expected hours: 20 – 40 per week
